Supported Living
Our Approach
First of all, we are ambitious for the people we support: we want them to transition into independent living successfully,
to stay well, and to avoid readmission to long term or hospital care.
From their first assessment, we will work with each person at their own pace,
listening carefully to their needs, hopes and dreams and helping to establish their goals.
We will develop a bespoke pathway together with clear, but sensitively agreed milestones to help plan for positive risk taking,
exciting new experiences, new skills and new stages in their lives.
We want to empower people to develop within a supportive environment where they have the security and comfort of slowing things down
if they need to or to work progressively towards their own independent flat or house in the community as that becomes a growing and possible reality for them.
We also firmly believe that supported living provision should be based on needs rather than diagnosis,
recognising that services designed around a particular clinical presentation can become institutionalised and do not represent current best practice.
Our management team is highly experienced in assessing and placing people according to their compatibility rather than clinical diagnosis.
This assessment methodology has been tested, established and proven to be both cost efficient and very effective in supporting people with differing needs and social backgrounds to live communally,
maintain tenancies and transition successfully into independent living.
Our homes offer safety, not through risk aversion, but by selecting the right mix of tenants with the very best support team that can work with each individual to complete their pathway to independence.
We achieve this by having a strong clinical input which far surpasses typical supported living models.
Our Service
We specialise in working with people with: Learning Disabilities, Autistic Spectrum Disorders, Mental Health Issues.
Many people referred to our services also have co-morbidities and our investment in a strong clinical team ensures that we are well placed to deliver safe care across a wide range of related health conditions.
We are able to support people with: Acquired Brain Injuries and Genetic Conditions, such as Huntington's Disease and Duchenne Muscular Dystrophy.
We have successfully improved outcomes for individuals with Forensic Mental Health Conditions, resulting in a reduction in hospital readmission.
